1. I hereby declare that the Petitioner was in a qualifying relationship with the deceased the late Antoine Daniel Adonis also known as Daniel Adonis and as such she is a surviving partner to a qualifying relationship within the meaning of Article 731 and 745 of the Civil Code of Seychelles, 2020
